On the day of survey March 05, 2012, based on review of facility records and documentation of fire sprinkler inspections and testing, it was determined the facility was not in compliance with the requirements of NFPA-101-LSC, and NFPA-25-Care, Maintenance, and Testing of Automatic Fire Sprinkler Systems.
NFPA-25 chap.-1-9---Inspection and testing of sprinkler systems and components shall be inspected at intervals specified in Table 2-1.
Chap. 1-10.1---All components shall be tested and verified that they function as intended.
Chap. 2-2.6----Alarm devices shall be inspected quarterly (every three (3) months) to verify that
they are free of physical damage.

The Findings Include:

1. Record review determined that sprinkler inspections were not performed quarterly as required.
2. The last documented sprinkler system inspection was performed on November 1, 2012.
3. There was no inspection performed on the next due date of February 2012.
4. There was no current documentation of inspection and testing of the system control valves
and supervisory switches.
6. The wet sprinkler systems and the dry sprinkler systems shall be inspected, tested, and
documented as required by NFPA-25.
