Based on documentation review and staff interview, it was determined that the hospital failed to conduct quarterly fire drills on each shift.

Findings include:

1. During a review of the hospital fire drill records on 1/05/2015 at approximately 1:45 p.m., no record could be found of fire drills being conducted for the third quarter and second and third shift for fourth quarter of 2014.

2. During an interview with the maintenance director on 1/05/2015 at approximately 1:50 p.m., it was revealed that the above fire drills were not conducted during the third and fourth quarters.

Findings include:

1. During a review of the hospital range hood records on 1/05/2015 at approximately 2:45 p.m., no record could be found of the range hood extinguishing system being inspected for the first half of 2014.

2. An interview with the hospital maintenance director on 01/05/2015 at approximately 2:50 p.m., revealed that a biannual inspection of the rangehood extinguishing system had not be conducted for the first half of 2014.
