Based on observation and interview the facility failed to provide emergency task illumination for the emergency generator in case of power failure and generator failure. This deficient practice affects all occupants in the facility. The facility census was 18 patients.

Findings included:

1. Observation on 06/19/12 at 8:50 AM during a tour of the facility showed the area where the emergency generator is located was not provided with a battery emergency light of at least 1 and ? hour duration to provide task illumination in the event of mechanical failure of the generator.

2. Staff E, Manager Facilities Service, confirmed at that time a battery emergency light unit was not provided for the generator.

Section 3-4.2.2.2 of the National Fire Protection Association (NFPA 99) states emergency battery light for task illumination for emergency power at the generator set location shall be provided.

Based on observation and interview the facility failed to maintain all sprinkler heads in a manner that would ensure the proper operation of the sprinklers during a fire. This deficient practice affects all occupants of the lower level of the facility. The facility census was 18 patients.

Findings included:

1. Observation on 06/19/12 at 9:30 AM during a tour of the facility showed three escutcheon rings missing from three ceiling mounted sprinkler heads in the lower level data room.

2. Staff E, Manager Facilities Service, confirmed at that time the three escutcheon rings were no longer in place around the three ceiling mounted sprinkler heads.

Section 6.2.7.2 of the National Fire Protection Association (NFPA 13) states escutcheons used with recessed, flush-type, or concealed sprinklers shall be part of a listed sprinkler assembly.

Section A.6.2.7.2 of the National Fire Protection Association (NFPA 13) states use of the wrong type of escutcheon with recessed or flush-type sprinklers can result in severe disruption of the spray pattern, which can destroy the effectiveness of the sprinkler.
