Based on policy review, staff interview observations during a tour of the facility and review of the medication refrigerator temperature logs, it was determined that the facility failed to ensure that drugs were appropriately stored at the correct temperatures.
Tour of the pharmacy on 5/2/2016 at 14:00 with the Pharmacy Director, and review of the medication refrigerator logs revealed that the medication refrigerator temperature was out of stipulated range ( 2° to 10° Celsius) as follows :-
· April 2016 (11 of 31 days= 37%),
· March 2016 (7 of 31 days=23%),
· February 2016 (8 of 29 days=28%),
· January 2016 (2 of 31days=6.5%),
· December 2015 had 0 days,
· November 2015 (1 of 30 days=3%).
Interview with the pharmacist on 5/2/2016 at 2:25 PM in the pharmacy confirmed the above finding.
Review of policy titled "REFRIGERATION DRUG STORAGE," dated 9/21/15 revealed that "All refrigerated drug storage areas will be inspected daily to ensure compliance with drug storage standards. "
