Based on review of the Grievance Response Log and interview, it was determined the facility failed to assure written responses to persons submitting grievances included any decision made regarding the grievance, the steps taken to investigate the grievance, the results of the grievance process or the date of completion. The failed practice did not assure persons filing grievances were informed of the outcome or resolution of the grievance and had the potential to affect all patients admitted to the facility. Findings follow:

A. Review of the Grievance Response Log from 05-01-13 through 05-31-13 revealed grievances were responded to in a form letter which did not include a decision made regarding the grievance, the steps taken to investigate the grievance, the results of the grievance process or the date of completion.

B. During an interview with the Interim Manager for Emergency Services on 07/01/03 at 1340, she verified the findings.
