Based on observation, it was determined that the hospital failed to ensure that the patient areas were maintained in a safe and sanitary manner.
Findings were:
In the patient shower area adjacent to the nurse ' s station, missing tiles were observed on the floors and the walls. Mold was noted growing in several spots between tiles.
In the Respiratory Rehabilitation Area, a soiled pillow was found stored amongst respiratory supplies and equipment.
Underneath the sink in ER Patient room # 1, the surveyor found 6 unsecured bottles of hazardous chemicals/cleaners.
The patient bathroom in ER Patient room # 2 contained 2 holes in the sheet rock behind the toilet. In that same bathroom, there was a wall heater with a rusted surface that made disinfection impossible. Underneath the sink in ER Patient room # 2, the surveyor found 5 unsecured bottles of hazardous chemicals/cleaners.
In the casting area, 1 of the six drawers in the room contained a hazardous chemical/cleaner.
In an interview with the Facility Administrator on 2/24/10, the condition of the patient shower area was confirmed. The Facility Administrator stated, " I was just made aware of the condition of the shower room and have a work order in to fix it. "

Based on review of documentation, it was determined that the Governing Body failed to enforce its own rules and regulations.
Findings were:
Facility policy entitled, " Crash Cart Inspection " stated that the " nursing staff at shift change (one per 24 hours) will do a 24 hour defibrillator, suction machine, oxygen and laryngoscope " check.
On 2/24/10 the crash cart, located in the patient care area, had no documented evidence of inspection since 2/22/10.
The lack of documented inspection of the crash cart was confirmed by the Facility Administrator on 2/24/10 at approximately 3pm in the conference room.
