Based on observation and staff interview the facility failed to ensure proper separation of hazardous areas from other spaces. This deficient practice would allow for the spread of smoke and fire to travel into the adjacent area, affecting 4 residents in 2 of 8 smoke zones. The facility has a capacity of 25 and a census of 4.
Findings include:
During the tour on 8/20/12 between 9:00 AM and 2:00 PM the following is observed:
--1) There is no self closure on the 1st floor oxygen storage room.
--2) There is no self closer on the 2nd floor dirty liner closet.
Staff A was present and confirmed the findings.
NFPA Standard: Protection from any area having a degree of hazard greater than that normal to the general occupancy of the building or structure shall be provided by one of the following eans:
(1) Enclose the area with a fire barrier without windows that has a 1 hour fire resistance rating in
accordance with Section 8.2.
(2) Protect the area with automatic extinguishing systems in accordance with Section 9.7.
(3) Apply both 8.4.1.1(1) and (2) where the hazard is severe or where otherwise specified by Chapters 12
through 42. 2000 NFPA 101, 8.4.1

Based on observation and staff interview the facility failed to ensure proper connection of smoke detectors in the basement corridor to the fire alarm system.This deficient practice would allow for the spread of smoke and fire to travel undetected by upper floors into the adjacent area, affecting 4 residents in 1 of 8 smoke zones. The facility has a capacity of 25 and a census of 4.
FINDINGS INCLUDE:
During the tour on 08/20/12 between 9:00 AM and 2:00 PM the following is observed:
1) The basement is found to have battery powered smoke detection that is not connected to the fire alarm panel.
Staff A was present and acknowledged this finding.
NFPA Standard: 2-1.2.2 Automatic and manual initiating devices shall contribute to life safety, fire protection, and property conservation only if used in conjunction with other equipment. The interconnection of these devices with control equipment configurations and power supplies, or with output systems responding to external actuation, shall be detailed elsewhere in this code or in other NFPA codes and standards.

Based on observation and staff interview the facility failed to ensure power strips are not being used as permanent wiring. This deficient practice could cause an electrical fire or the equipment to fail in the event the equipment overloads the capacity of the power strip, affecting 4 residents in 2 of 8 smoke zones. This facility has a capacity of 25 and a census of 4.
Findings include:
During the tour on 8/20/12 between 9:00 AM and 2:00 PM the following is observed:
--1) There is a extension cord power a light fixture plugged into a power strip in Physical Therapy.
--2) There is a block multiplier found powering a refrigerator and equipment in Physical Therapy.
Staff A was present and confirmed the finding. Staff A stated the multiplier would be replaced with a new 4 plug oulett and extension cord will be removed from light fixture..
NFPA Standard: Flexible cords and cables shall not be used: as a substitute for the fixed wiring of a structure; run through holes in walls, ceilings or floors, doorways or windows; attached to building surfaces; or concealed behind building walls, ceilings, or floors. 1999 NFPA 70, article 400-8
