Exit Signage

Based on observation and interview, it was determined the facility failed to maintain illuminated exit signage affecting 1 of 7 exit discharges within the component.
Findings include:
1. Observation on October 25, 2017, at 10:40 AM, revealed the 1st floor South West stairwell lacked an exit sign.
Exit interview with the facility representatives #1 and #2 on October 25, 2017, between 2:00 PM and 2:15 PM, confirmed the needed exit sign.
Sprinkler System - Maintenance and Testing

Based on documentation review and interview, it was determined the facility failed to maintain the automatic sprinkler system, affecting one of three levels within the component.
Findings include:
1. Observation on October 25, 2017, between 12:00 PM and 12:20 PM revealed:
a. 12:00 PM - The 1st floor Elevator room near X-ray (which had the suspended ceiling removed) lacked upright sprinkler heads.
b. 12:20 PM - The basement level corridor, outside Central Supply, had a group of wires attached to the sprinkler piping
Exit interview with the facility representatives #1 and #2 on October 25, 2017, between 2:00 PM and 2:15 PM, confirmed the sprinkler deficiencies.
