Based on observations, interviews, and record reviews, the facility failed to ensure that all foods were labeled as to when they were placed in the refrigerator.

Findings included:

Observations of the facility kitchen on 06/24/14 at 2:00 PM revealed the following: Frozen tater tots, potatoes, carrots, sausage, onion rings, chicken, bread, bagels, french toast, pancakes, bacon had been placed in the facility freezer without being dated as to when they were placed in the facility freezer.

Interview on 06/24/14 at 2:15 PM with Food Service Staff #1 revealed the listed frozen food items needed to have been dated by dietary staff prior to being placed in the facility freezer. Food Service Staff #1 threw the above listed food items in the trash.

Record review of facility dietary policies and procedures, not dated, revealed the following: "date perishable foods prior to storage; refrigerated storage: cover, label, date, and store above raw foods."
