Facility failed to maintain sprinkler system in accordance with the Standard for Fire Sprinkler Installation, NFPA 13. Findings included the following:
Surveyor observed between 1:30 and 2:00 pm on 11/13/13 in the second floor equipment storage room that 2 fire sprinkler heads were missing escutcheon plates. This is not in accordance with NFPA 13, 1996: 2-2.5.2. Escutcheon plates used with recessed or flush-type sprinkler shall be part of a listed sprinkler assembly.

Surveyor observed between 1:30 and 2:00 pm on 11/13/13 that field paper test of air flow relationship indicated air flowing out of laboratory to corridor rather than inward from corrior to laboratory.
