NFPA 90A (1999 Edition), 2-3.4.1 A service opening shall be provided in air ducts adjacent to each fire damper, smoke damper, and smoke detector. The opening shall be large enough to permit maintenance and resetting of the device.
NFPA 90A (1999 Edition), 2-3.4.2 Service openings shall be identified with letters having a minimum of 1/2 in. (1.27 cm) to indicate the location of the fire protection device(s) within.
NFPA 90A (1999 Edition), 3-4.6.1 The locations and mounting arrangement of all fire dampers, smoke dampers, ceiling dampers, and fire protection means of a similar nature required by this standard shall be shown on the drawings of the air duct system.
Based on observation, the facility failed to maintain the smoke dampers as evidenced by service openings in the air ducts that were not identified as having fire protection devices within the air duct, a damper that was closed and the failure to provide drawings of the air duct system showing the locations of the dampers. This affected 4 of 4 smoke compartments in the Unit and had the potential for dampers not to be tested and maintained and damper failure.
Findings:
During the facility tour with Staff 1 and 2 on June 2, 2010, the dampers were observed.
At 10:25 a.m., mechanical damper #3 in the smoke barrier adjacent to rooms 246 and 247 in Unit 410 was closed. Staff 1 confirmed the linkage was disconnected.
At 10:25 a.m., 3 of 3 damper service openings in the smoke barrier adjacent to rooms 246 and 247 were not identified as having dampers inside the ducts. Staff 1 confirmed the service openings did not identify the dampers inside the air duct.
At 10:30 a.m., Staff 1 and 2 were asked if drawings were available showing the locations and mounting arrangements of the dampers in the air duct system. Staff 1 and 2 stated none were available.
