Based on records review, the facility failed to inspect fire doors annually. The deficient practice affected two (2) of two (2) smoke compartments, with the potential to affect all residents, staff, and visitors. The facility has the capacity for 16 beds with a census of 12 on the day of the survey.

Record review on 9/23/24 at 1:30 PM, revealed the facility could not provide documentation of an annual inspection and testing of fire doors.
