Based on observations and staff interview the facility failed to ensure the security of the medication cart and the medication room for one of four medication storage rooms in the facility.

The findings include:

1) On 9/26/11 at 10:15 AM, a tour of the Adolescent Unit revealed the door of the medication room was propped open with the medication cart, which was not in use at that time. The medication cart was observed to be unlocked and contained drawers labeled with patient names and medications inside. The center cabinet above the sink in the medication room was found unlocked and contained stock medications such as Milk of Magnesia, EC ASA and liquid Depakote among other medications. The Charge Nurse accompanied the surveyor on the tour of the medication room and confirmed the medication room and the medication cart were required to be locked when not in use.

2) A review of the policy and procedure for sensitive security areas provided by the facility revealed that access to sensitive areas and key control procedures will be strictly enforced in these areas.

3) On 9/27/11 at 1:30 PM, an interview with the Infection Control Nurse/Quality Improvement Coordinator revealed secure areas to be locked included the medication room. The interview revealed the facility has several areas which were monitored by video surveillance and the medication room was not under video surveillance range.
