Based on observation, record review, and tour with facility staff on the day of survey October 4, 2011, the facility was not in compliance with the requirements of NFPA-101-Life Safety Code, NFPA-13 Automatic Fire Sprinkler Systems, and NFPA-25- Care and Maintenance of Automatic Fire Sprinkle Systems.
The facility failed to maintain existing protection and life safety features to prevent the spread of fire and the passage of smoke in event of an emergency.
NFPA-101-LSC chapter 4.12.1 Maintenance and Testing.
Whenever or wherever any device, equipment, or level of protection is required for compliance with the provisions of this code, such device, equipment or level of protection shall thereafter be continuosly maintained in accordance of applicable NFPA requirements.
The findings include:
Concealed sprinklers with temperature sensitive 135 degree F. ceiling plates have been removed, painted, and made inoperable at:

6. The 100 wing room #114 bathroom.
7. The 400 wing room #401.

10. The lobby rest room.
