Based on record review, staff interview and policy and procedure review, the facility failed to have all medical records completed within 30 days following discharge.
Findings Include:
Record review revealed 71 incomplete records over 30 days from the facility's Emergency Room (ER) and 61 incomplete records from the facility's inpatient floor.
Review of the facility's "Delinquent Medical Records" policy revealed, "... letters will be sent out each month to the physician with delinquent medical records. (The only records counted as delinquent is over thirty days). If records are not completed within the prescribed time frame, the physician's admitting priveledges will be temporarily suspended until the records are completed."
On 8/13/2014 at 10:15 a.m. the Medical Records Director stated, "Some of the physicians are having problems with the electrnic record. One (1) physician has been suspended."
During exit conference on 8/13/2014 at 2:15 p.m. these findings were disclosed. No additional documentation was provided.
