Based on review of facility documents, medical records (MR), and staff interview (EMP), it was determined the facility failed to ensure that a registered nurse monitored patient, post urinary catheter removal for one of one medical records reviewed. (MR)
Review of policy entitled "... Urinary Catheter Insertion, Maintenance and Post Urinary Catheter Care", dated August 2020, revealed "... 12. Patient monitoring and interventions post urinary catheter removal will follow the Post Urinary Catheter Protocol (Addendum B) unless specified differently per provider order. ... Addendum B Post Urinary Catheter Protocol Discontinue Foley Catheter - Patient has not voided within 6 hours - Bladder Scan - < 400 ml continue to monitor patient for voiding or signs of discomfort or urinary retention Q 6 hrs PRN > 400 ml Straight Cath - Continue to monitor patient for voiding signs of discomfort or urinary retention Q 6 hrs PRN ... ."
1. Review of MR1, revealed patient had foley catheter removal on February 21, 2024, at 11:07 PM, and had not voided. There is no documentation that the patient had a bladder scan completed six hours of foley catheter removal.
2. Interview with EMP2 on May 28 2024, confirmed that a bladder scan should have been completed on February 22, 2024, at 5:00 AM.
