It was determined by observation during the course of the survey, December 23, 2014, the facility failed to install and maintain the automatic fire sprinkler system in accordance with NFPA 13, Installation of Sprinkler Systems and NFPA 25, Inspection, Testing and Maintenance of Water-Based Fire Protection Systems.
During the walk through of the facility with the Maintenance Director, the fire alarm test connection (Inspector's test) did not have an orifice equivalent to one sprinkler head type. The orifice size found during the survey was one (1) inch in diameter. 5-15.4.2
