Based on observation and interviews, the CAH failed to meet the requirements of the Condition of Participation of the Physical Plant and Environment by failing to ensure that it was constructed, arranged, and maintained to ensure access to and safety of patients, and provide adequate space for the provision of services. This deficient practice is evidenced by failing to ensure a call bell system was available for 5 of 5 rooms in the ED (see findings in C-0910).

Based on observations and interviews, the CAH failed to ensure the physical environment was maintained for the safety of the patients. This deficient practice is evidenced by failing to ensure a call bell system was available for 5 of 5 rooms in the ED.
Findings:

Observations made in the ED on 08/08/2022 revealed there were no call bells located in any of the 5 patient rooms.

On 08/08/2022 at 11:35 a.m., an interview was conducted with S1EDRN. He stated the ED had a wireless push button alert system. He stated it consisted of one call bell device which could be given to only one patient at time.

On 08/09/2022 at 1:15 p.m., an interview was conducted with S2DON. He confirmed there was only one push button call device for the 5 beds in the ED. He stated they needed to update the call bell system so every patient in the ED could have a call device.
