Based on observation, and interview the facility failed to maintain a safe setting in patient rooms as evidenced by the presence of shower stalls with flaked paint, wall board and /or tile in rooms for 7 of 10 rooms observed (#s 103, 105, 106, 107 ,108, 110 and 111).
Additionally patient bedrooms with missing caulking to seal the floor tile in the personal showers of 11 of 13 rooms observed (#'s 407, 411, 412, 413, 607, 608, 609, 610, 612, 614 and 615).

During a tour of the facility 4/29/24 at 11:25 am with Staff #8 Clinical Nursing Manager several rooms on the second floor were observed.
Observations during the tour were:

Room 407 Rusty bottom, wall heating/ air conditioning (A/C) unit Rust falling off in flakes, sharp edges on sides, chipping paint.

Room 411 Rusty bottom, wall heating/ air conditioning (A/C) unit Rust falling off in flakes, sharp edges on sides, chipping paint.

Room 412 Rusty bottom, wall heating/ air conditioning (A/C) unit Rust falling off in flakes, sharp edges on sides, chipping paint.

Room 413 Rusty bottom, wall heating/ air conditioning (A/C) unit Rust falling off in flakes, sharp edges on sides, chipping paint.

Room 607 Rusty bottom, wall heating/ air conditioning (A/C) unit Rust falling off in flakes, sharp edges on sides. Shower stall calking missing/ shrink causing gap between floor and wall.

Room 608 Rusty bottom, wall heating/ air conditioning (A/C) unit Rust falling off in flakes, sharp edges on sides. Shower stall calking missing/ shrink causing gap between floor and wall.

Room 609 Rusty bottom, wall heating/ air conditioning (A/C) unit. Shower stall calking missing/ shrink causing gap between floor and wall.

Room 610 Wall heating/ air conditioning (A/C) unit Rust sharp edges on sides. Shower stall calking missing/ shrink causing gap between floor and wall.

Room 611 Wall heating/ air conditioning (A/C) unit Rust sharp edges on upper surface and sides.

Room 612 Wall heating/ air conditioning (A/C) unit Rust sharp edges on sides. Shower stall calking missing/ shrink causing gap between floor and wall.

Room 614 Extremely rusty and corroded rusty, wall heating/ air conditioning (A/C) unit rust falling off in flakes.

Room 615 Wall heating/ air conditioning (A/C) unit Rust sharp edges on sides. Shower stall calking missing/ shrink causing gap between floor and wall.

When interviewed 4/30/24 at 11:50 am, Staff #5 Director of Plant Operations stated he does environmental rounds monthly along with the maintenance technicians. He stated he looks for walls that need to be painted including touch-ups and torn mattresses. He stated they have not been looking at room in wall heating/ air conditioning (A/C) units or shower tiles and confirmed the multiple rusty in wall heating/ air conditioning (A/C) units and missing shower tile caulking.
