Based on observation, it was determine that, the facility failed to ensure doors to storage rooms are self closing.
Findings include:
1. On 6/8/12 at 10:55 AM, in the presence of Staff #36, it was observed that the door to the OR orthos storage room could not close.

Based on ovservation, it was determined that, the facility failed to ensure Alcohol Based Hand Rub dispensers (ABHR) are not installed above ignition sources.
Findings include:
1. On 6/8/12 at 10:20 AM, in the presence of Staff #36, an ABHR was observed to be installed above an electrical outlet in Bay #4 of the Recovery Room.
