Based on observation and a tour of the facility, the infection control officer failed to develop a system for identifying and controlling infections and communicable diseases of patients and personnel. Failure to do so could result in an increase of infections and communicable diseases in patients due to airborne bacteria, hard surfaces that cannot be properly cleaned and sanitized and infestation of disease-carrying pests.
Findings were:
During a tour of the patient care area on 12-10-14, 3 of 5 patient rooms visited (rooms #236, #303 and #305) were found with dust on high horizontal surfaces. These rooms were cleaned, vacant and ready for patient use.
During a tour of the kitchen on 12-10-14, 4 of 5 kitchen drawers examined contained small, loose, dried crumbs of residue that appeared to be dried food. Located in 1 of the 4 drawers was an ice-cream type scoop that had not been thoroughly cleaned and was still caked in a dried substance that appeared to be dried food.
During a tour of the kitchen on 12-10-14, the walk-in refrigerator contained a rusty shelf, prohibiting it from being properly cleaned and sanitized.
The above information was confirmed in an interview with the Chief Executive Officer, the Chief Nursing Officer and the Chief Operating Officer on the afternoon of 12-10-14 in the facility conference room.
