Based on observation, and interview, it was determined the facility failed to provide measures to prevent and control the spread of infection and maintain a clean and sanitary environment in that they failed to keep environmental surfaces clean from white and rust color buildup on the sink spouts and sinks in the Emergency Department (ED). This failed practice had the likelihood to affect all patients receiving care, and all staff and visitors to the ED. Findings follow:
A. During a tour of the Emergency Department, observation showed the following:
1) On 09/30/24 at 2:00 PM, three of the five treatment rooms had a heavy buildup of white residue on the spout of the sink. The sink had a white and rust color residue from midway to the drain of the sink. The ED Manger acknowledged the findings during the tour.
2) On 10/01/24 at 9:00 AM, the trauma and cardiac room sinks had a heavy buildup of white residue on the spout of the sink. The sinks had a white and rust color residue inside the sink.
B. The ED Manger verified the findings at the time of observation.
