Bringing transparency to federal inspections

Based on findings from observations and interview, several locations in the Pratt building did not comply with the 2000 NFPA 101 requirements for protection of hazardous areas.
Findings include:
-- Per observation during a tour of the 4th floor Alternative Level Care (ALC) unit on 12/2/14 at 1:45 pm, the trash room across from room 4143 lacked a 1-hr fire rated door.
-- Per observation on the ALC unit on 12/2/14 at 2:00 pm, the room labeled "Linen Only" across from room 4137 had mobile carts with approximately 40 gallon bags of soiled linens in each cart as well as a large trash transport cart containing five more bags of soiled linens. However, the room did not have a 1-hr fire rated door and also lacked a self-closing device.
Per interview of the Director of Facilities on the ALC Unit at 2:00 pm, renovations were completed on this floor during a prior construction project, and the room labeled "Linen Only" was originally designed for other purposes (not requiring fire rated protection). He/She acknowledged that the change in use of the room, now as a soiled linen storage room, resulted in requirement for a 1-hr fire rated and self-closing door.
-- Per observation during a tour of the 3rd floor Physical Medicine and Rehabilitation (PM&R) unit on 12/2/14 at 2:15 pm, the Dirty Utility Room across from room 3132 lacked a self-closing door.
