Based on medical record reviews, and staff interviews, physicians' orders for admission and discharge to the observation rooms, inpatient rooms, and swing bed rooms lacked either complete dates or times the orders were written. This involved 5 of ten sampled patients' medical records (Patients #1, 3, 4, 5, and 6). The facility census during this visit was 12.
Findings include:
On 08/23/12, a review of ten patients' medical records revealed the lack of a complete date or time of when the physician wrote an order to admit or discharge to/from the observation rooms, inpatient rooms, and swing bed rooms lacked either complete dates or times the orders were written. This involved 5 of ten sampled patients' medical records (Patients #1, 3,4 ,5, and 6). This was verified in interviews with Staff D and E, on 08/23/12, between 2:23 PM and 3:55 PM. These medical records are as follows:
Patient #1 was admitted to a swing bed on 02/12/12 for pneumonia care. A discharge order was written on 02/27 to discharge the patient to a nursing home. The physician's order lacked the year and a time the order was written. The referral for continuity of patient care form, for transferring the patient from the inpatient unit to swing bed, was signed by the physician; however, the signature lacked a date of when it was signed. The signatures by two registered nurses were signed on 02/15/12 for this form.
Patient #3 was admitted had a physician's order dated 06/06/12 for admission to swing bed for pneumonia care. The order lacked a time the order was written. Although the physician signed the referral for continuity of patient care form, for transferring the patient from the inpatient unit to swing bed, this form lacked a date of when the physician signed it. Two registered nurses signed the form on 06/06/12.
Patient #4 was admitted on 03/02/12 for post-operative care for a right knee replacement. The patient was admitted to a swing bed for therapy, and was discharged on 03/12/12. The physician's order for discharge lacked a time the order was written.
Patient #5 was admitted to a swing bed for care of a right foot ulcer on 06/19/12. Although the physician signed the referral for continuity of patient care form, for transferring the patient from the inpatient unit to swing bed, this form lacked a date of when the physician signed it. Two registered nurses signed the form on 06/19/12.
Patient #6 was admitted to a swing bed on 07/03/12. A physician's order for the discharge from inpatient unit to swing bed lacked a time when the order was written. Although the physician signed the referral for continuity of patient care form, for transferring the patient from the inpatient unit to swing bed, this form lacked a date of when the physician signed it. Two registered nurses signed the form on 07/03/12.
