SUPERVISION OF CONTRACT STAFF

Based on interview and record review, the licensed nurses providing services in the facility failed to adhere to the policies and procedures of the hospital for 3 (Patients #19, 20, and 21) of 5 patients reviewed for hygiene, resulting in the potential for less than optimal outcomes. Findings include:

On 9/29/2022 at 1055, review of Patient #19's face sheet revealed he was a 67-year-old male who presented to the emergency room on 09/26/2022 at 1855 and was admitted as inpatient on 09/26/2022 at 2104. Patient #19 was a current inpatient at the time of the medical review being held in the emergency department. Review of the complete record with Staff AA (Clinical Informative), revealed no documentation of hygiene or bathing for Patient #19.

On 9/29/2022 at 1045, review of Patient #20's face sheet revealed he was a 75-year-old male who presented to the emergency room on 04/09/2022 at 1618 and was admitted as inpatient 04/11/2022 at 2340. Review of the patient's medical record with Staff AA (Clinical Informative) and Staff U, revealed no documentation of hygiene or bathing for Patient #20 during that time.

On 9/29/2022 at 1045, review of Patient #21's face sheet revealed he was a 54-year-old female who presented to the emergency room on 02/13//2022 at 1155 and was admitted as inpatient on 02/16/2022 at 1630. Review of the patient's medical record with Staff AA (Clinical Informative) and Staff U, revealed no documentation of hygiene or bathing for Patient 21 during that time.

On 09/29/2022 at 1300, review of facility Staff Nurse-ED Summary Description dated 10/08/21 revealed General Duties: 4. Complies with approved polices, procedures, guidelines, and standards as established by the facility and its associated facilities.

On 09/29/2022 at 1150, review of facility Acute Care Provisions of Care policy, dated 4/1/2020, revealed under section: PROVISIONS: Implementation/Interventions; 4. Each patient receives appropriate hygiene measures.

In an interview on 09/29/2022 at 1150 with Staff F (Associate Chief Nursing Officer), she stated it was her expectation that all patients should be getting their hygiene provided, including the patients who are boarded as inpatients in the emergency department. Staff F stated the nursing staff need to get it documented.
