Based upon observations and staff interviews on June 18, 2014 between approximately 0830 and 1230 hours Snoqualmie Valley hospital has failed to maintain fire resistive construction of the building capable of resisting the passage of smoke and fire into other compartments. This could allow the toxic product of combustion to move out of a room and into the exit access corridor and the smoke compartment which would endanger the residents, staff and/or visitors within the facility.
The findings include, but are not limited to:
1) Hole through stub wall in Nurse's station near copier, allowing an energized power cord to pass through wall.
2) Hole through wall in Penthouse from mechanical room to IT room allowing multiple energized extension cords to pass through wall.
The above was discussed and acknowledged by the Maintenance Director.

Based upon observations and staff interviews on June 18, 2014 between approximately 0830 and 1230 hours Snoqualmie Valley hospital has failed to maintain corridor walls so that they will resist the passage of smoke. This could allow the toxic product of combustion to move out of a room and into the exit access corridor and the smoke compartment which would endanger the residents, staff and/or visitors within the smoke compartment.
The findings include, but are not limited to:
1) Hole in corridor wall under desk in Medical Records office.

Based upon observations and staff interviews on June 18, 2014 between approximately 0830 and 1230 hours Snoqualmie Valley hospital has failed to maintain doors without impediments to their closing and latching. This could result in a delay in getting the door to the room closed in the event of a fire. This could result in toxic products of combustion getting into the room and into the exit corridor which would endanger the residents, staff and/or visitors within the smoke compartment.
The findings include, but are not limited to:
Two corridor doors failed to close and latch, the locations are:
1) Soiled utility room door, Pharmacy hallway.
2) Doctor's sleep room, pharmacy hallway.
The above was discussed and acknowledged by the Maintenance Director.

Based upon observations and staff interviews on June 18, 2014 between approximately 0830 and 1230 hours Snoqualmie Valley hospital has failed to assure proper maintenance of the facilities portable fire extinguishers. This potentially delays a quick response to contain a fire from spreading which could expose and endanger residents, staff and/or visitors within the facility.
The findings include, but are not limited to:
1) Halon PFE in IT room, located in the Penthouse is in need of annual testing.

Carbon Monoxide Alarms
Group I or Group R occupancies shall be provided with single station carbon monoxide alarms installed outside of each separate sleeping area in the immediate vicinity of the bedrooms in dwelling units or sleeping units and on each level of the dwelling. The carbon monoxide alarms shall be listed as complying with UL 2034 and be installed and maintained in accordance with NFPA 720-2012 and the manufacturer's instructions
Exceptions:
1. For other than R-2 occupancies, the building does not contain a fuel-burning appliance, a fuel-burning fireplace, or an attached garage; or
2. Sleeping units or dwelling units in I and R-1 occupancies and R-2 college dormitories, hotel, and DSHS licensed boarding home and residential treatment facility occupancies which do not themselves contain a fuel-burning appliance, or a fuel-burning fireplace, or have an attached garage, need not be provided with carbon monoxide alarms provided that:
a. The sleeping unit or dwelling unit is not adjacent to any room which contains a fuel-burning appliance, a fuel-burning fireplace, or an attached garage; and
b. The sleeping unit or dwelling unit is not connected by duct work or ventilation shafts with a supply or return register in the same room to any room containing a fuel-burning appliance, a fuel-burning fireplace, or an attached garage; and
c. The building is provided with a common area carbon monoxide detection system.
3. An open parking garage, as defined in Chapter 2 of the International Building Code, or enclosed parking garage ventilated in accordance with Section 404 of the International Mechanical Code shall not be considered an attached garage.
IFC 908.7 2012 / WAC 51-54A
Based upon observations and staff interviews on June 18, 2014 between approximately 0830 and 1230 hours Snoqualmie Valley hospital has failed to provide CO detection in the following locations:
1) Main lobby, near the gas fireplace.
2) Downstairs in the corridor outside the Boiler room.
3) Weyerhaeuser Conference room adjacent to the Boiler room.
The above was discussed and acknowledged by the Maintenance Director.

Based upon observations and staff interviews on June 18, 2014 between approximately 0830 and 1230 hours Snoqualmie Valley hospital has failed to restrict the use of daisy-chained multi-plug outlets (power strips) to providing power to permitted electrical equipment. This could result in a fire from overheating of the plug strip due to the heavy power draw endangering the residents, staff and/or visitors within the facility.
The findings include, but are not limited to:
1) Emergency Room Doctor's office
2) Medical Records office, UPS plugged into a multiplug adapter
3) IT office, downstairs
