Based on record review and interview, the facility failed to complete individual comprehensive treatment plan for six of ten current patients ' sample (ID #s 5, 6, 7, 8, 9 & 10).
Findings:

Review of records on 3/12/13 revealed that written treatment plan was not initiated for:
? Patient ID# 5 admitted 3/8/13
? Patient ID# 7 admitted 3/5/13
? Patient ID# 10 admitted 3/5/13

Interdisciplinary treatment plan was not completed for the following patients:
? ID# 6 admitted 3/5/13
? ID# 8 admitted 3/8/13
? ID# 9 admitted 3/7/13

Medical records reviewed with the Nurse Educator who acknowledged that the treatment plans should have been completed. She added that the facility requires that initial treatment plan be completed by the nurse on admission, and the interdisciplinary treatment plans be completed by the Interdisciplinary Treatment Team within 72 hours of admission.
