Based on facility policy review and interview, the facility failed to ensure patient rooms were terminally cleaned by 2 staff members (Technician #1 and Technician #2) of 4 staff interviewed on the terminal cleaning process (intense disinfection procedure) of patient rooms.

The finding included:

Review of facility policy titled "Terminal Cleaning" effective date 6/2018, revealed "...Rooms...will be terminally cleaned prior to admission of another patient to that room...Spray approved germicidal solution on sink, wipe sink and all fixtures...Spray approved germicidal solution on shower...Spray approved germicidal solution on commode. Wipe entire commode and all fixtures...Sweep floors...Use microfiber mop (saturated in approved germicidal solution) to mop the entire floor..."

Interview with the Director of Plant Operations on 10/22/19 at 1:30 PM, in the Administration Conference Room, revealed it was the facility's policy for all patient rooms to be terminally cleaned after a patient was discharged and prior to admission of a new patient to the patient room. Terminal cleaning included mopping the floors and cleaning the bathrooms and between the hours of 4:30 PM and 7:00 AM the terminal cleaning was performed by nursing staff.

Interview with Technician (Tech) #1 on 10/22/19 at 2:30 PM, in the Administration Conference Room, confirmed he did not always mop the floors and clean the bathrooms after patients were discharged and before new patients were admitted to the room.

Interview with Tech #2 on 10/22/19 at 2:42 PM, in the Administration Conference Room, revealed he did not always have time to mop the floors and clean the bathrooms when terminally cleaning rooms after patients were discharged. Continued interview confirmed patients were admitted to patient rooms that had not had the floors mopped or the bathrooms cleaned.

In summary, the facility failed to ensure patient rooms were properly cleaned after a patient was discharged and prior to the admission of a patient to the same room. The facility failed to follow facility policy.
