Based on record review, observation and interview the facility failed to uphold their policy related to maintaining a clean environment in the emergency room.
Findings include:
Review of facility policy titled "Safety for Environmental Services Staff, revised 07/2022, Cleaning of the Emergency Room (ER): stated
Clean all surface areas with approved germicidal, clean stretcher pad top and bottom, clean side rails and frames. Refill paper products and soap as needed. Replace sharp containers when level is reached. Clean machines in room, discard trash, check bio-hazard boxes and remove as needed. Empty linen hamper daily. Dust and damp mop. Stretchers are terminally cleaned once every two weeks or as needed.
Observation of the ER on 12/20/2022 at 1140 revealed dusk in the various ER rooms, including light figures, cardiac monitors, and supply carts (rooms 1,2,3, 4, 6,10, and 11). The medication room was noted to have dusk on top of the Omnicell, and diapers on the floor. Chipped paint was noted on the walls in room 10 and 11. Rust was also noted on stretchers in various rooms (1,2, 6, 10, 11). The two faux leather chairs used by staff, revealed the seats of both chairs peeling, exposing the cloth lining.
Interview with staff (ID #51) during observation verified the finding noted in the ER.
