Based on observation, testing, and staff interviews it was determined the facility failed to maintain doors in corridors capable of resisting fire for at least 20 minutes, having no impediment to closing and latching the doors. This could place all Emergency Room patients at risk in the event of a fire in the facility.
The findings were:
During a tour of the facility with Staff M on 01/12/12 from 10:00am to 12:00pm it was observed by testing corridor smoke doors in the Emergency Room Corridors that the doors did not latch creating a smoke resistance barrier.
These findings were confirmed by Staff M at the time of discovery.

Based on observation, review of facility records, and staff interviews it was determined the facility failed to ensure that all staff members take part in quarterly fire drills.
The findings were:
During a review of facility records on 01/12/12 at 4:00 pm it was determined the facility failed to conduct fire drills for the third shift during the second and third quarters of 2011.
These findings were confirmed by Staff A at the time of discovery.
