Based on observation and interview the facility failed to ensure two of three fire/smoke walls observed were able to resist the passage of smoke or fire due to the presence of penetrations. The facility census upon entrance was four Acute Care patients, zero Swing Bed patients and one Observation (Observation - outpatient services provided to a patient while the patient's physician decides whether to admit the patient to Acute Care services or to discharge the patient) patient for a total census of five.
Findings included:
1. Observation on 02/14/24 at approximately 9:00 AM of a wall near zone E1 showed one two inch in diameter hole in the wall above the ceiling. The wall near zone C1 had two one half inch in diameter holes.
2. During an interview on 02/14/24 at approximately 9:00 AM, Staff I, Facility Services Supervisor, confirmed the walls were not smoke tight due to the presence of holes which had not been repaired.
