Based on observation and interview, it was determined four (fourth floor Patient Room 4001 and third floor Patient Room 3001, 3012 and 3022) of six smoke barrier doors in the Main Hospital failed to fully close when released from the magnetic hold open device. The failed practice had the potential to affect all patients, staff due potential of smoke passage from one smoke compartment to another if the smoke barrier doors did not fully close in the event of a fire event. The facility had a census of 143 patients on 06/23/14. The findings follow:

A. On a tour of the facility on 06/25/14 at 1330 with the Plant Operations Director, the following smoke barrier doors did not fully close when released from the magnetic hold open device:
1) The set of smoke barrier doors located on the Fourth Floor near Patient Room 4001.
2) The set of smoke barrier doors located on the Third Floor near Patient Room 3022.
3) The set of smoke barrier doors located on the Third Floor near Patient Room 3012.
4) The set of smoke barrier doors located on the Third Floor near Patient Room 3001.
B. The Plant Operations Director verified the smoke barrier doors did not fully close during the observation.
