Hazardous Areas - Enclosure

Based on observation and staff interview on 2/17/17, it was determined the facility failed to ensure that all hazardous areas are maintained properly fire and smoke protected.

Findings include:

1. During a tour of 183 South Orange Avenue conducted at 10:30 AM, in the presence of Staff #55, it was found that within electrical room #D1556 there was an unprotected sprinkler pipe penetration and the partition wall was not visually fire stopped at ceiling level.

2. During a tour of 671 Hoes Lane conducted at 1:00 PM, in the presence of Staff #56, the following were noted:

a. A room identified as being used for storage within the information services department did not have installed the required smoke resistant door.

b. Unprotected wall penetrations which could allow the migration of smoke and fire within the building were found located in the following areas:

(i) Inside electrical room #C-122E.

(ii) Within the D Building housekeeping storage room adjacent to the kitchen and laundry area.

3. These findings were confirmed by Staff #55 and Staff #56.

Based on observation and staff interview on 2/16/17, it was determined the facility failed to ensure that electrical safety be maintained at all times.

Findings include:

Reference: NFPA 70, National Electrical Code 2017 Edition 110.26 states: "Spaces about Electrical Equipment. Access and working space shall be provided and maintained about all electrical equipment to permit ready and safe operation and maintenance of such equipment..."

1. During a tour of 671 Hoes Lane conducted at 10:30 AM, in the presence of Staff #56, a large amount of stored cardboard like material and boxes were found directly piled against electrical equipment inside the information services back-up power room.

2. This finding was confirmed by Staff #56.
