MAINTENANCE OF PHYSICAL PLANT

Based on surveyor observations, it was determined that the hospital failed to ensure that the physical condition of the psychiatric unit was maintained in a manner to ensure the safety and well being of patients.

Findings are as follows:

During a tour of the West 4 Dual Diagnosis Psychiatric Unit on 3/9/11 at approximately 9:15 AM with the Risk Manager, Director of Patient Safety, Director of Behavioral Health, and Nurse Manager, the following conditions were observed that pose hazards for patients at risk of harming themselves:

A television located in the Group Therapy/Quiet Activity Room with an unsecured electrical cord approximately 16-18 inches from the back of the television, plugged into an electrical wall outlet.

Lever door handles on all doors, with the exception of the patient rooms.

"Drop" ceiling tiles in the Group Therapy/Patient Activity Room, common hallway, and nursing station.

In addition, during a tour of the West 3 Geriatric-Psychiatric Unit on 3/9/11 at approximately 10:50 AM, the following conditions were observed that pose hazards for patients at risk of harming themselves:

A television located in the Group Therapy Room with an unsecured electrical cord approximately 16-18 inches from the back of the television, plugged into an electrical wall outlet.

"Drop" ceiling tiles throughout the unit, including patient rooms, with the exception of the Group Therapy Room.

Open door hinges throughout the unit, including patient rooms, with the exception of the Dining Room.

Sinks in patient bathrooms with lever type faucets.
