Based on observation and interview with the Director of Plant Operations, it was determined that the facility failed to maintain the fire/smoke barrier walls to provide their required fire/smoke resistance rating. A fire/smoke barrier that is not maintained would in the event of fire allow fire, smoke and toxic gasses to migrate into adjacent smoke compartments, prolonging exposure to the buildings occupants. This poses potential for adverse affects to health and safety.

The findings include:

1. During the tour of the facility (West Kendall), on 5/19/11 at 1:45 p.m. it was observed in the IT room several unprotected openings passing thru the fire/smoke barriers. This consisted of wires and pipes that were unprotected. The director of plant operations stated staff will conduct a facility wide inspection to correct any other possible deficiencies.

2. During the tour of the facility (Doral Center), on 5/19/11 at 3:45 p.m. it was observed in the IT room several unprotected openings passing thru the fire/smoke barriers. Based on observations and staff interview the facility failed to provide exit access from the building without the use of a special tool or knowledge from the egress side. This condition could delay or deny access to the smoke proof towers in the event of a fire, endangering the building occupants.

Findings include:

It was observed with the Director Of Plant Operations at the main hospital campus on 5/18/11 at 3:30 p.m., that the exit door leading to the stairwell on the third floor near room 372 had a 15 second delayed egress locking device. The door not did have the sign posted on the door indicating how the function works.

Based on documentation and observation on 5/19/11 at Palmetto Bay failed to provide a properly tested and maintained fire alarm system. The deficient practice would affect all smoke compartments, all residents and staff.

Findings include:

The facility at 11:45 a.m. was unable to produce the sensitivity report for the smoke detectors. This was confirmed by the Director Of Plant Operation at the exit conference.

Based on observations and staff interviews conducted at the Main Hospital during the survey on May 18, 2011 it was determined that this facility did not comply with specific requirements of NFPA 70, the National Electrical Code and the NFPA 99.

Findings include:

During the facility tour at 2:30 p.m. it was observed in the Office Of Support Staff Office that an extension cord was in use. The equipment in use was various computers and office equipment. Extension cords are not permitted as permanent wiring.

Means of Egress - General

During the facility tour with the Facilities Director of Plant Operations on May 19, 2011 it was observed that a number of installed alcohol based hand sanitizer dispensers were mounted next to or over electrical outlets, switches and ignition sources not allowing a minimum 4" clearance as required by NFPA 101 (2000 Edition) 19.3.2.5 (6): " The dispensers shall not be installed over or directly adjacent to an ignition source."

The findings include:

During the tour of (West Kendall) building it was observed in the recreation room several hand sanitizer dispensers mounted next to light switches. This condition was observed in several areas thru out the building. The facility needs to inspected all of these hand sanitizer dispensers in the facility and insure they are installed in the proper location.
