Based on direct observation, the hospital failed to ensure floors on the patient care units were thoroughly cleaned and free of dirt and debris. Failure to ensure cleanliness in areas where patients receive care creates an environment that fosters the presence of bacteria and the potential development of infections.

Findings include:

The second and third floor patient care units were observed on June 14, 2012. A flooring seam was noted in the doorways of each patient room where two different flooring materials met. The seams varied in width from a fraction of an inch to approximately 1 inch wide. Although the seams appeared to have been previously covered, the coverings had worn away and had left a sticky residue.

The seams were noted to be caked with significant amounts of dirt and debris and had not been thoroughly cleaned or covered to prevent the possible transmission of bacteria.
