They were the following issues.
A) The sprinkler system was not provided at some rooms such as IT closets and electrical closets.
B) There were numerous escutcheon plates for the sprinkler heads were missing.

The inspector observed, while accompanied by CEO, COO, Director of infection control, chief operation officer, Plant operation coordinator, Business development manager, Material management personnel, Biomedical personnel, during the hours of the inspection from 10:00 AM to 6:30 PM on 08/01/2019 that there were the following issues. They were the following issues.
(A) There was a missing receptacle powered from life safety branch at the automatic transfer switch location and generator set location. There was a missing battery powered operated light units at the generator set location.

The inspector observed, while accompanied by CEO, COO, Director of infection control, chief operation officer, Plant operation coordinator, Business development manager, Material management personnel, Biomedical personnel, during the hours of the inspection from 10:00 AM to 6:30 PM on 08/01/2019 that there were the following issues. They were the following issues.
(A) There was a missing receptacle powered from life safety branch at the automatic transfer switch location and generator set location. There was a missing battery powered operated light units at the generator set location.
(B) The electrical directory was not updated in accordance to the new or modified receptacles.
(C) There were flexible conduits utilized from emergency electrical outlet for crash cart at the corridor. This wiring system shall be a nonflexible metal raceway.
(D) Receptacles were not tested or logged. Electrical Systems and Receptacles were not tested or logged for the isolation monitor inside the OR. -Receptacles not listed as hospital-grade at these locations are tested at intervals not exceeding 12 months. The facility shall establish a testing interval and maintain testing records.
(E) The elevator light fixture shown to be powered from the critical branch. Please verify and correct it back to the life safety branch. The CAT Scan receptacle shown to be powered from the life safety branch. Please verify and correct it back to the critical branch.
(F) The automatic powered operated door shown to be powered from the normal branch. Please verify and correct it back to the life safety branch.
(G) The receptacles for the ice machine and the refrigerator shown to be powered from the normal branch. Please verify and correct it back to the critical branch.
(H) The labels provided for the electrical panels and the essential transfer switches were not readily identifiable to identify life safety, critical and equipment branch. This is not in accordance with NFPA 70.
(I) Please verify that there is a bonding wire provided between the normal and essential branch-circuits panelboards. This does not comply with NFPA 70, 2002: 517-14. - Panelboard bonding requires the equipment grounding terminal buses of the normal and essential branch-circuits panelboards serving the same individual patient vicinity be bonded together with an insulated continuous copper conductor not smaller than # 10 AWG.
(J) There was no air conditioning provided for portion of the hospital such as OR suite and the OB suite and some office. Supply, return, and exhaust ventilation system for surgical and obstetrical delivery suites, intense care, coronary care, nursery, and emergency treatment care spaces shall be operational at all time and generated from the generator emergency system. Due to the air condition not operational mold and mildew were presented at portion of the hospital such as OR suite and the OB suite and some office causing infection control concern. The air quality was a concern as well. NFPA99, 2012: 6.4.2.2.5.4. (4).
