Based on record review and interview, the facility failed to transmit all necessary medical information at the time discharge to the appropriate past-acute care service provider. This deficiency is evidenced by the failure of the facility to notify the accepting facility of significant findings on the skin assessment at the time of discharge/transfer in one (#5) of two (#5, #6) records reviewed for transfer of medical information to an accepting facility.

Review of the Multidisciplinary Notes of Patient #5 revealed, on 11/29/2021 at 11:00 a.m."Report called to RN Kyra at Assumption Healthcare at (985)369-6011.".

Review of the Skin Assessment and Wound Care documentation revealed on 11/29/2021 at 7:30 p.m. the documentation of a large new bruise to the right inner thigh and groin. Further review of the nursing notes and skin assessment revealed the nurse failed to notify the family or the accepting nursing home of the new skin findings.

In telephone interview on 12/07/2021 at 8:20 a.m., S4RN verified that she had documented the lesion and that she did not notify the accepting facility of the new findings.

In interview on 12/07/2021 at 11:10 a.m., S2DON verified that the accepting facility should have been notified of the new finding discovered while the patient was waiting for transfer.
