Based on document review and interviews, the hospital failed to complete background checks before the first day of employment for two (2) of ten (10) new hire records reviewed.
Findings:
During the review of new hire records on 12/9/2025 and 12/10/2025, the background check for CNA #1 who started work on 12/1/2025 was not completed until 12/4/2025. The background check for CNA #2 who started work on 12/1/2025 was not completed with until 12/4/2025.
This was confirmed by the HR Operations Specialist at the time of the finding. On 12/10/2025 at 9:20 AM, the Senior Vice President & General Counsel confirmed that the background check needs to be completed before the start of work.
