Based on observations and review of logs, the facility failed to assure that portable fire extinguishers were checked on a monthly basis.
The findings include:
In accordance with Section 39.3.5 of NFPA 101 Life Safety Code which states: Portable fire extinguishers shall be provided in every business occupancy in accordance with Section 9.7.4.1.
In accordance with Section 9.7.4.1 of NFPA 101 Life Safety Code which states: Where required by the provisions of another section of this Code, portable fire extinguishers shall be installed, inspected and maintained in accordance with NFPA 10, Standard for Portable Fire Extinguishers.
Personnel shall make monthly inspections of the fire extinguishers and shall keep records that show the date the inspection was performed and the initials of the person completing the inspection per sections 4-3.4.1, 4-3.4.2 and 4-3.4.3 of NFPA 10, 1998 edition.
The inspection logs and individual tags on each extinguisher were reviewed and observed during the course of the survey on January 29, 2013. Neither the logs nor the individual tags attached to the extinguishers denoted that a monthly check of the extinguishers was conducted during 2012 or so far in 2013.

Based on observation, the facility failed to protect the means of egress from any impediment which could restrict the full instant use in cases of emergency.
Findings include:
In accordance with 39.2.1.1 of NFPA 101, Life Safety Code, all means of egress components shall be in accordance with Chapter 7 of this Code.
In accordance with 7.1.10.1, of NFPA 101, Life Safety Code, all means of egress shall be continuously maintained free and clear of all obstructions or impediments to full instant use in the case of fire or other emergency.
The building is a 40 feet by 60 feet building with parking along the south side of the building. The main entrance/exit is at the front of the building. The secondary exit from the main floor is along the east side of the building through an office space. The exit for the upper level is also at the south side of the building.
The parking area and means of egress were examined at 8:30 a.m. on January 29, 2013. There were no parking blocks or bollards to keep vehicles a safe distance away from all means of egress from the front and east side of the building and also near the egress stairs which serves as a means of egress for the second level.
