Based on observation of the facility, review of facility documentation and staff interview and verification, during the post survey revisit, it was determined the facility failed to ensure the environment was maintained in a manner safe from fire. The facility failed to ensure the ambulatory health care occupancies were separated from other tenants and occupancies by fire barriers with at least a 1 hour fire resistance rating. The facility also failed to ensure one exit access was arranged to ensure a continuous surface between the exit discharge and the public way.(V710) The facility had a capacity for 1061 patients. The cumulative effect of these systemic practices resulted in the facility's inability to ensure the safety of all patients in the facility.

The facility had a capacity for 1061 patients This had the potential to affect all patients, visitors and staff .
