Based on review of policies and procedures, review of other information provided, tours of the hospital and outpatient areas, and interviews with key staff March 19-21, 2012, it was determined that the facility failed to ensure that the CAH was maintained to ensure access to and safety of patients at all times.
Findings include:
1. The ceiling tiles were scraped and edges contained loose particles of the tile in ICU and Med/Surg units.
2. There was a water stained ceiling tile in the nurses ' lounge in ICU.
3. There was no signage that indicated the location of eye wash stations in the OB/GYN and Pediatrics outpatient areas.
4. A metal ladder and a cloth netting were too close to the electrical breaker located in the Rehabilitation Services building.
5. These findings were confirmed by hospital staff.

Based on a tour of the dietary area and interviews with key staff on March 21, 2012, it was determined that the facility failed to ensure that the nutritional needs of inpatients were met in accordance with recognized dietary practices consistently.
Findings include:
1. Two dented cans were found in the food storage room.
2. The maple pork from the previous day ' s lunch was not labeled in a legible manner.
3. These findings were confirmed by hospital staff.

Based on a tour of the outpatient areas and interviews with key staff on March 21, 2012, it was determined that the facility failed to maintain the confidentiality of record information and provide safeguards against loss, destruction, or unauthorized use consistently.
Findings include:
1. During tour of Skowhegan Family Medicaine, the Office Manager stated that the old records were kept in totes in the cellar.
2. Looking through the double hung windows of the cellar, the surveyor observed four (4) totes, labeled with patient names on the side of the container.
3. This finding was confirmed by the Office Manager on March 21, 2012.
