Based on observation and interview, the provider failed to maintain the smoke tight rating of corridor wall assemblies for three randomly observed corridor doors (the men's locker room, women's locker room, and staff lounge) were not equipped with positive latching hardware. Findings include:

1. Observation revealed the corridor doors to the men's locker room, women's locker room, and staff lounge were not provided with positive latching hardware. Those doors only had a push-pull and door closer. Interview with the maintenance supervisor at the time of the observation confirmed those findings. He state the doors had been like that since the original 2002 construction.
