Based on document review, observations and interviews the facility staff failed to ensure patients were provided information on who to contact to file a grievance.
The findings include:
On 8/8/12 during the tour of the facility the lobby/waiting area was observed. The facility did not have posted the information informing patients the name address and toll free telephone number of the State Agency (Department of Health; Office of Licensure and Certification) that has the licensure responsibilities for the facility.
Also on 8/8/12 a review of the documents given to the patient at the time of registration did not have the State Agency information.
The facility Administrator stated, "We will have that corrected."
