Facility failed to maintain access to exits in accordance with the 2000 edition of the Life Safety Code, NFPA 101. Findings included the following:
Surveyor observed between 10:00 and 11:00 on 2/06/13:
1. exit signage at Sabine building, 2nd floor ICU led to mag locked doorway connection to Trinity Tower which did not release upon initiation of fire alarm in accordance with NFPA 101, 2000: 7.10.1.4.

Surveyor observed between 10 and 11 am on 02/06/13 that corridor walking surface outside the MRI in the first floor of the Sabine Building was not in accordance with NFPA 101, 200: 7.1.6.

Surveyor observed between 10 and 11am on 02/06/13 that ceiling tiles were stained in teh central supply room in the first floor of the Necheds buiding.

Facility failed to maintain cooking equipment protection in accordance with the Standard for fire Extinguishers, NFPA 10. Findings included the following:
Surveyor observed between 11 and 11:30 am on 02/06/13 that one of two "K" type extinguishers were not properly placarded to read "fire protection system shall be activated prior to using fire extinguisher" in accordance with NFPA 10:4.3.2.2.

Facility failed to perform electrical receptacle testing in accordance with the Standard for Health Care, NFPA 99. Findings included the following:
Surveyor observed between 2:00 and 2:30 pm on 2/06/13 that facility records do not include receptacle testing in patient care areas in accordance with NFPA 99:3-3.3.3 and 7-6.2.1.2. Receptacles shall be tested in Patient Care areas as follows:
Phyisical integrity of each receptacle (visual)
Continuity of the grounding of each receptacle (test)
Correct polarity of hot and neutral connections in each receptacle (test)
The retention force of the grounding blade shal be not less than 4 oz. (test)
Testing intervals:
General care areas tested annually
Critical care areas tested semi-annually
wet location tested annually.

Surveyor observed between 11 and 11:30 am on 2/06/13 that in the dietary department, two of four electrical panels, labeled "KA" and "KD" were not properly secured in locked position in accordance with NFPA 70:110-17.
