Based on observations and interview, the facility failed to ensure all means of egress was free of obstructions. This deficienct practice had the potential to expose patients, staff, and visitors to possible harm in the event of an emergency. Findings:


Observations during the facility tour on 9/28/2011 revealed medical carts blocking the means of egress in both the Emergency Room Hallway and the in-Patient Hallway.

Based on document review and interviews, the facility failed to ensure all emergency lights were tested within the requirements of the code on an annual basis. These deficienct practice had the potential to expose patients, staff, and visitors to a potential smoke or fire environment.
Findings:


Review of documentation and through conducted interviews during the facility tour on 9/28/2011 revealed that there was no 90-minute annual testing performed on any of the facility's emergency lights.


These observations were acknowledged by the Facility Maintenance Manager.
