Based on observation, interview, and policy review the facility failed to maintain all kitchen floor covering in an orderly, easily cleanable condition. The failure to keep the kitchen clean poses a health risk to all patients that receive food from the kitchen. The facility census was 11.
Findings included:
1. Record review of the facility's policy titled "Sanitation," dated 01/14, directed staff to mop the kitchen floor daily.
2. Observation on 09/24/14 at 2:05 PM, in the facility kitchen, showed 11 eight inch square floor tiles missing in a room adjacent to the kitchen housing one refrigerator, two freezers and one combination refrigerator/freezer.
3. During an interview on 09/24/14 at 2:06 PM, Staff B, Food Service Manager, confirmed that these floor tiles were missing and the floor can not be easily cleaned.
