Based on staff interview, personnel record review and facility policy review, it was determined that one (1) of two (2) registered nurses (RN's), who was a contracted nurse, failed to receive unit specific orientation and competency evaluation.

The findings include:

On 2/8/22, a review of selected employee personnel records was conducted. The review revealed Employee #2, a contracted RN with a start date of 10/25/21 and a last work date of 1/23/22, had completed the new hire classroom and computer training, however there was no evidence that any unit training or orientation or an evaluation of unit orientation proficiency was completed.

On 2/8/22 at 3:10 p.m. an interview was conducted with the Chief Operating Officer (Staff #2). Employee #2's personnel record was discussed. Staff #2 explained Employee #2 was a contracted nurse through their corporate contracted staff agency and had a thirteen (13) week contract with the facility. When asked for evidence that Employee #2 received training or orientation to the hospital units, Staff #2 stated "It isn't there, it was missed." A request for the facility policy regarding orientation was requested and received.

Hospital policy titled "Hospital Policy on Clinical Services #723.07 with the last revision date of 09/2020 read in part:

"...2. All new employees of the clinical services department shall receive an orientation to the hospital and to the requirements of their job prior to working with patients independently.
3. All clinical staff will be oriented to policy, procedures, unit routines, behavior programs, etc. necessary to provide appropriate patient care....
5. The clinical services orientation shall include...and the competency skills checklist...
7. The clinical services orientation, through preceptorships, shall be the responsibility of the Clinical Supervisor in each discipline...
11. Preceptors are responsible for reviewing with the orientee policies, procedures, unit routines, patient care, patient safety, behavior management approaches, etc., as outlined in the unit orientation checklist and for signing off orientation checklist..."
