Based on observations during the tour of the facility on the afternoon of 5/30/2013, with the Director of Life Safety Risk Manager and Plant Maintenance Manager, the facility failed to provide direction to an exit. There were several doors along a corridor that did not lead to an exit. The one door that did continue the egress path did not have an exit sign. Therefore the travel to an exit was unclear at 1) at first floor clinic building near its elevators and 2) at client housing, outside of mechanical room 1100.

All grounding testing had been conducted annually, however the facility must maintain these records on it is facility.
