Based on observation and interview it was determined the facility failed to provide a clean and odor free environment.
The findings include:
Observations on 04/12/11 at 12:40 PM revealed strong, foul urine odors in the dayroom, Nurses Station and the men and women's restrooms across from the Nurses Station on unit Gragg 1. The restrooms had a dried brown substance, wet toilet tissue and gritty, grimy substance on the floors. The dayroom floor had dust bunnies of various sizes around the perimeter of the room.
Observations on the ISU unit revealed large dust bunnies along the wall on the left side of the unit door. The shower had a dried brownish substance on the walls and floor and the chairs in the dayroom were grimy.
Observations on units Wendell 1, 3 and 4 revealed strong, foul urine odors in the restrooms and the hall near the restrooms. The floors in the restrooms were grimy and the showers/tubs were grimy with a dried brownish substance.
Interview on 04/12/11 at 12:50 PM with the Administrator revealed the housekeeping was performed by a contracted cleaning company. The facility did not have any housekeeping staff. She stated it was good to have new people in the facility, to point out what the staff did not see because they had become accustomed to it.
Interview on 04/13/11 at 09:30 AM with Patient #11 revealed the bathrooms on unit Gragg 1 had odors and frequently had urine on the floors. The patient had not seen the staff mop the floors in the dayroom, halls or their bedroom for approximately seven to ten days.
Interview on 04/14/11 at 1:45 PM with Unsampled Resident #15 revealed the bathrooms on unit Wendell 4 had odors "that would knock you down" and always had mold.
Interview on 04/14/11 at 3:55 PM with the Director of Housekeeping revealed the housekeeping staff worked eight hours on day shift, one on each unit and they did what they could within those parameters. He had additional staff to clean the offices. The staff were to clean the tubs, showers and floors every day. The floors in the patients' rooms were to be spot cleaned daily, there should not be any dust bunnies. Interview further revealed the staff were to glance in the restrooms at least three times per day to ensure the areas were clean. Further interview revealed the two team leaders made two rounds daily to ensure the units were cleaned and the he made rounds with the Nurse Managers at least every month and by himself once or twice per week. He stated he could not add more staff on the units without the facility's approval because it would cost the facility more. He stated he probably needed to meet with the facility's Administration to focus on cleaning patient areas instead of the offices.
Interview on 04/15/11 at 12:30 PM with Licensed Practical Nurse (LPN) #1 revealed the rooms, floors, walls and bathrooms were dirty and the odors were bad on Wendell 4, they needed more staff.
Interview on 04/15/11 at 1:10 PM with Mental Health Associate #1 revealed more housekeepers were needed to clean the unit. He stated cleaning was part of his job however, with all of his other duties he did not have time to clean.
Interview on 04/15/11 at 3:30 PM with Floor Tech #1 revealed he had not had time to clean the "build-up" on the floors of Gragg 1, was too busy with other things, and needed more help.
Interview on 04/15/11 at 3:45 PM with Housekeeper #1 revealed she cleaned the shower on ISU the other day (Wednesday). She did not know how it got that way except the cleaning carts were not allowed on the unit and housekeepers were not allowed on the unit at times.
